This is a new install. I have created a service and it is running. I tested it at the ip/ArcGIS/rest/services/InterstateBuffers/MapServer?f=jsapi and it displays just fine.
When I try and create a web application, I fill out page one then go to specify the layer. In the add layer dialog all is OK until I select the service then all that happens is the progress bar in the lower left corner just moves and it will not do anything when I press add. No details are provided.
On what maybe a related issue. When I first setup the server I was able to get past this point. However when I got to the page where I could select one of the default color themes, the graphics with the colors did not display. Continuing I created an application but the layer did not display. The selected colr did display.
