If you open an MXD or SXD, and the program crashes, next time you open up ArcMap or ArcScene the last MXD isn't listed in the recent documents list under the > File menu.
It appears that documents are added to this list during the close_document event.
Why not add them to the list during the open_document event? That way they'll be available even if the program crashes.
